Dear Sir,
At the suggestion of many Officers, an important work is being prepared on the V.C. and the D.S.O., giving a complete and authoritative biography of all those who have been awarded these decorations, from the time of their institution to the end of the present War.
The editing of the work has been entrusted to me and I write to ask if you will kindly assist me by filling in the enclosed form with such particulars as it is desirable should be included, e.g., the school at which you were educated, details respecting your Military Career, Pursuits, Sporting Achievements etc., which may be incorporated in the biography.
The Official account of the action for which you won your D.S.O. will be included if details were given in the London Gazette, and as the publishers assure me that they will spare no expense to make the "V.C. and D.S.O." a standard and authoritative work, I hope you will help me as far as it is possible.
Many portraits will be given and in the case of subscribers the publishers have undertaken to reproduce a portrait in every case provided we can supply one where there is no copyright fee payable to the photographer.
I will have a proof of the biography submitted to you before publication.
