Instructions for use of this card, in 3rd Section.
1. - Units will indent on Adv. Depot Med. Stores, as follows:-
Cards.
Field Medical Units - each 300
Casualty Clearing Stations - each 1000
Field Laboratories - each 500
In addition, each Field laboratory will indent for 5 record books, each of 500 pages, in duplicate with the cards.
2. - Every sample sent to a Field laboratory, for examination for an infective disease the carriers of which may be detected by bacterioscopic methods, will be accompanied by a card duly filled up in the Field Medical Unit sending the sample.
3. - The card, having been filled in by the O.C. Field Laboratory will be returned to the Field Medical Unit, and will be sent with the patient on all transfers until it eventually reaches the M.O. of the patient's unit, by whom it will be noted and filed for reference in the event of the patient again reporting sick. If the patient be again sent to a Field Unit, his card should be sent by the R.M.O. to the O.C. of the Field Medical Unit and accompany the patient on all subsequent transfers.
